3M Filtek Supreme Ultra: A Clinical Review of a Nanofilled Universal Composite
Filtek Supreme Ultra is 3M's nanofilled universal composite, a direct restorative resin built on discrete silica nanoparticles and zirconia-silica nanoclusters and sold as a single material for anterior and posterior work. Available in 36 shades across four opacities, radiopaque and fluorescent, it is the composite many general practices reach for across most direct cases, which is why Filtek Supreme Ultra sits among the most widely stocked restoratives in the operatory. It belongs to the same class as the other universal composites in our guide to dental composites, and its filler chemistry is the right place to start when judging where it fits in a working tray.
The nanofiller behind the polish and the wear resistance
The filler combines non-agglomerated silica nanomers near 20 nm with loosely bound zirconia-silica nanoclusters that range from roughly 0.6 to 10 microns, at a filler load near 72.5% by weight in the universal shades. Under load the nanocluster behaves as one large reinforcing unit, while the primary particles are small enough to polish to an enamel-like luster. The clinically useful consequence is gloss retention: as the surface wears, more nanoparticles are exposed instead of large filler being plucked out, so the restoration holds its polish longer than a conventional microhybrid. In 3M's own toothbrush-abrasion testing the Filtek Supreme Ultra translucent shades kept their gloss ahead of a long list of hybrids and microfills after several thousand cycles, which matches the chairside reputation for polishing quickly and staying glossy. The resin matrix uses bis-GMA, UDMA and bis-EMA with reduced TEGDMA to limit shrinkage, and the shades are organised into Body, Enamel, Dentin and Translucent groups for layered esthetics.
Which restorations is Filtek Supreme Ultra indicated for?
The label covers any cavity class in both arches. Anteriorly it suits Class III, IV and V cavities, diastema closure and direct veneers, where the Dentin and Enamel shades allow polychromatic layering. Posteriorly the Body shades and the higher filler load carry Class I and II restorations under occlusal load, and the material also serves for core build-ups, splinting, and indirect inlays, onlays and veneers. What it is not is a bulk-fill: increments go in at 2 mm or less, so a deep proximal box takes longer than it would with a dedicated bulk-fill resin. It also does not replace a flowable base where one suits the preparation, and the Translucent shades will not mask a dark substrate on their own, which is a job for an opaquer or a dentin shade underneath.
Placing and curing Filtek Supreme Ultra
- Isolate the field and complete the preparation.
- Etch according to the chosen adhesive (selective-etch or total-etch) and apply the bonding agent to its instructions.
- Place the composite in increments no thicker than 2 mm, and 1.5 mm for darker or translucent shades.
- Adapt and sculpt each increment; the stiff, non-slumping consistency holds the anatomy under ambient light.
- Light-cure each increment for 20 seconds with an LED unit at adequate irradiance, exposing the full surface.
- Finish and polish to develop and lock in the surface gloss.
How long do Filtek Supreme Ultra restorations last?
The posterior survival record is the reason the material keeps its place in the tray. A five-year randomized split-mouth trial reported a 92% survival rate for the nanofilled universal restorative in load-bearing Class II cavities, with a mean annual failure rate of 1.6% and chipping, debonding and recurrent caries as the main failure modes (PMID 36410582). The material studied is sold as Filtek Supreme XTE outside the United States and shares the same nanofill platform. A separate clinical study rated 97% of Class I and II restorations clinically sufficient at 36 months, with no meaningful difference between LED and halogen curing (PMID 29080928). Together these results place Filtek Supreme Ultra among the better-documented universal composites for posterior load, and the manufacturer reports 15-year clinical performance data for the wider Filtek Supreme family.
Handling strengths and the trade-offs to expect
The strengths are practical and well attested by clinicians. The paste is sculptable without slumping, the Body shades blend well enough that a single shade often serves small to medium cavities, and polish retention holds up over years of service. One material covering most anterior and posterior cases keeps the restorative tray simple, part of why Filtek Supreme Ultra works as an everyday driver rather than a case-specific specialist.
The trade-offs are just as concrete. Placement is incremental, so there is no bulk-fill time saving in deep cavities. Some operators find the stiff paste tacky against hand instruments, and capsule users report the dispenser can keep extruding after the twist stops, wasting material. Like any resin composite it picks up extrinsic stain from coffee and tea over the years, which polishing manages but does not erase. On delivery, single-dose capsules cut cross-contamination and waste in lower-volume settings while syringes suit high-throughput placement, so the choice there is about workflow and control rather than the material itself.
Filtek Supreme Ultra against Estelite and the alternatives
Against Tokuyama's Estelite Sigma Quick, the split is filler design and feel. Estelite's supra-nano spherical fillers give a fast, uniform polish and a creamy, non-stick paste, while the nanocluster system in Filtek Supreme Ultra favours strength, packability and gloss retention, staying where the instrument leaves it.
