CaviWipes Review: Contact Time and Use

CaviWipes: A Clinical Review of Metrex's Surface Disinfectant Wipes
CaviWipes are a ready-to-use surface disinfectant wipe from Metrex, pre-saturated with a mix of quaternary ammonium compounds and isopropyl alcohol. In the operatory they handle the clinical contact surfaces that change between patients: chair controls, light handles, bracket trays and counters. CaviWipes belong to the wider set of surface disinfection products a practice stocks for infection control, and choosing between the formats starts with the chemistry and the contact time.
How CaviWipes work: dual quaternary ammonium and alcohol chemistry
The quaternary ammonium compounds disrupt microbial cell membranes, while isopropanol denatures proteins and gives a rapid kill before it evaporates. Together they make an EPA-registered intermediate-level disinfectant with a label spectrum that is bactericidal, virucidal, tuberculocidal and fungicidal against a defined organism list. Two products in the line need telling apart: the original wipes carry a 3-minute contact time, while CaviWipes1 are formulated for a 1-minute contact time against the listed organisms, and Metrex also offers a hydrogen-peroxide version, CaviWipes HP, with a 1-minute universal contact time. All share the CaviCide chemistry in a pre-saturated towelette.
Which contact time do you actually need?
This is the point that decides whether disinfection happens. Contact time means the surface stays visibly wet for the full label time. The original wipes need the surface wet for 3 minutes, so if it dries at one minute it has to be re-wet. CaviWipes1 are built for a 1-minute wet time, which fits real chairside turnover far better. The failure is silent: alcohol-containing wipes evaporate fast, so the labelled contact time is often missed between patients. Choose the product whose contact time the turnover can actually sustain, and re-apply whenever the surface dries before the time is up.
The surfaces CaviWipes are for, and where they are not
Under the Spaulding classification they are for noncritical surfaces, the environmental and clinical contact surfaces of the operatory. The correct sequence is clean then disinfect: one wipe to lift visible bioburden, a fresh wipe to disinfect, because organic soil inactivates the chemistry. Each wipe covers one surface and is then discarded, since a used wipe can move organisms rather than remove them. What they are not for: they do not sterilise semicritical or critical instruments, which need heat sterilisation; they are not for skin or mucosa; and the alcohol can craze or cloud some acrylics, polycarbonate and certain plastics over repeated use, so material compatibility on upholstery and acrylic covers is worth checking.
Do disinfectant wipes actually work?
The lab and field data say yes, with technique caveats. A multicentre laboratory evaluation using the ASTM E2967 wipe standard found pre-soaked disinfectant wipes achieved a greater than 4-log10 reduction, above 99.99%, of Staphylococcus aureus and Acinetobacter baumannii within 10 seconds of wiping, though only one formulation prevented transfer of bacteria to a clean surface (PMID 26518272). That result is the clinical case for the one-surface-per-wipe rule. A controlled field study in a hospital setting reported pre-impregnated wipes reduced surface microbial bioburden more effectively than a cloth soaked in chlorine solution, and that wipe disinfection with staff training lowered recovery of multidrug-resistant organisms (PMID 29759641). Both come from general healthcare settings, but the surface-disinfection principles carry straight to the operatory.
CaviWipes compared with other surface disinfectants
Against CaviWipes1, the difference is the 3-minute versus 1-minute wet time, and the shorter time is usually easier to honour through a busy day, which is why the high-turnover operatory tends to standardise on it. Against accelerated hydrogen peroxide wipes, the trade is contact time and material compatibility: the peroxide chemistry tends to offer shorter contact times, less odour and gentler material effects, and it was the accelerated hydrogen peroxide product that prevented bacterial transfer in the wipe study above. Against quaternary-only wipes with no alcohol, the contrast is speed for gentleness, since the alcohol-free products are kinder to plastics but slower to act.
CaviWipes suit a practice that wants a proven intermediate-level disinfectant wipe and can hold the 3-minute wet time reliably; where turnover is fast and that wet time is hard to maintain, CaviWipes1 or the HP version fit better. Keep the canister sealed so the wipes do not dry out, and standardise every operatory on the same contact time to cut missed wet times. Either way, technique decides the outcome more than the product: clean first, keep the surface wet for the full contact time, and use one wipe per surface.
